Why the client sought assistance, what is urgent, and what is currently at risk.
Relevant circumstances contributing to the problem, including relationship changes, health issues, employment changes, business issues, legal matters, family violence, gambling, or other complexity.
Summary of income, expenses, debts, assets, liabilities, and overall position, with reference to the SoFP.
Any indicators affecting safety, decision-making, comprehension, stability, or ability to act. Note relevant cultural or access considerations where appropriate.
Protective factors, family or community supports, motivation, capacity, and any positive steps already taken.
What the client wants to achieve, preferably in their own words. Also note any outcomes the client has indicated they are not willing to pursue.
What was discussed, what advice was given, referrals made, and agreed next actions.
